I am a OASAS approved provider who will work with you to complete a Comprehensive Alcohol/Substance Abuse Evaluation. This may be needed for court-ordered evaluations due to DWI, family or criminal court, as well as Department of Transportation (DOT) Substance Abuse Professional (SAP) Evaluations.

The goal of the evaluation is to work with you and determine if there are problems with substances and, if so, make a recommendation to provide treatment options.

Your personal information will never be shared with anyone unless you first provide me with a release of information signed by you. Your results will be delivered only to you and, if applicable, whichever parties have your permission.
